Elder Care Management

Caring for a disabled, aging, or declining family member or friend can be frightening. You may be unsure where to turn for help and feel ill-equipped to make decisions regarding your loved one’s care. As an elder care manager and a medical professional who also specializes in end of life, Dr. Jan Zislis of ModernMedical acts as a guide and advocate for families caring for older relatives or disabled adults, whether they live locally or a long distance away.

Dr. Zislis provides families with a specialized focus on issues related to aging and elder care. With his dedicated coordination of treatment, and the expertise of the professionals with whom he collaborates, your loved one can get the care needed to age gracefully while attaining and maintaining his or her maximum functional potential. Our goals are to minimize your friend or family member’s risks of illness or injury, and to avoid needless emergency room visits if an issue should arise.

For those nearing end of life we understand that every person deserves to approach the end of his or her life in a humane and dignified way. Dr. Jan Zislis will be your advocate, providing advice, alternatives, and a comfortable process for end of life care.

Although the term can be confusing, “end of life care” refers not only to the healthcare of patients in the final hours or days of their lives, but also more broadly to the care of all those with a terminal illness or terminal condition that is becoming more advanced, progressive and incurable.

In the final stages of a life-limiting illness, it can become evident that in spite of the best care, attention and treatment, your loved one is approaching the end of his or her life. The patient’s healthcare continues during this time, although the focus shifts to making the patient as comfortable as possible. Whether you have received the news that you are approaching the end of your life, or you have learned that a loved one is facing this challenge, it is normal to feel anxious, depressed and overwhelmed. A full end of life care plan will focus on providing the best care possible, coordinating with specialists such as hospice and home nurses, and working through the decisions that must be made. Dr. Zislis takes a holistic approach, caring for both the patient and the caregiver, and providing professional advice on how to proceed through each stage of the end of life.
